Tele 5way wiring help

Hey guys.
The other I made a thread about very clean sounding pickups for a friends tele, but now he needs help with a wiring.
Hope you guys can give me a hand.

Here's what he wants:

1: Neck
2: Neck out of phase
3: Both parallel
4: Both Series
5: Bridge

He wants to do this with a standard 5 way switch, can this be done? Don't a need a super 5way for this?
Any suggestions are welcome.
It would be great if someone could provide the schematic.

I don't think you can do the out-of-phase thing with a standard 5-way, but don't quote me on that just yet.

Just give me a day or two. Any other wiring dudes, feel free to jump in.
