The controls you are looking for is just a minor modification of the Jazz Bass. I have used this schematic before, but I have always used 3 on/off switches instead of a 5-way switch. The problem with a 3-way or 5-way switch is you only have one output.

With a 5-way switch your taking three inputs and aggregating them down to one output as labeled in this diagram. With mini toggles you have three inputs and three outputs to feed the controls like in the first diagram. I don't know enough about multi-function switches to know how to do what you want?
