Trouble is, I've done this quite late at night. I'm pretty sure this will work, but at the same time I think it could be simplified.
Don't start soldering until someone else (ie Artie) has been in the thread and had a look!
I've used lots of different colour wires there. The really pale blue represents white wires, but the other colours of wires that aren't connected to pickups are just like that so that the diagram is a bit easier to follow.
EDIT: updated to fix mistake (do a hard refresh on the page to see it). I also recommend some heatshrink on the legs of the capacitor.

Haha, I've immediately spotted a problem with it having woken up this morning! Will get it fixed ASAP.

I used Visio to do the diagrams, and some stencils I downloaded from TDPRI.

Thinking about it, although it looks fine on the diagram, you might not want to use the capacitor to go from the switch to the tone pot, if it's a longish way. If the legs reach then great, so long as you heatshrink them. If not, just take a wire from the switch to the pot, and put the capacitor between the middle lug of the pot and the pot casing.



This is the version with those recommendations done. I think this is probably a better version.
