Which diagram should I use ??

I have installed the pups and I know I messed em up in pos 4 I get an out of phase sound from the descriptions I have read here. Which wiring diagram should I use I have a H-S-H setup, one tone and one volume pot and a 5 way switch. I dont want to do any thing fancy and I just want to make sure I used the right schematic. I have an Ibanez RG 270 DX and I am new to installing pups. I used the SD diagram for HUM-SING-HUM, vol, tone 5 way with split.

PS RG I havent had time yet to switch the wireing to test your advice yet I apologize for that.

No Infecto X this post was previous to the other one. I did the wiring for mine and it was ok.
